Noncardiogenic pulmonary edema is an "increased-permeability" edema. Injury or inflammation disrupts the alveolar-capillary membrane, allowing protein-rich fluid to leak into the interstitium and alveoli even though left-sided heart pressures and pulmonary capillary wedge pressure remain normal. In contrast, cardiogenic edema is driven by elevated hydrostatic pressure from left-ventricular dysfunction, often producing cardiomegaly and vascular congestion on imaging.
